The Orlando Trip is now officially approved and ready to start taking payments.
Please read and fill out the attached document to secure your student a place on the trip. Chaperones are needed! Please consider joining us on the trip.

Medical forms
In order to carry ANY medications you must have a form filled out and turned in to the EPMS Music Staff. There are two forms.
SBH-1 allow a student to have over the counter medications (advil, etc) AND/OR Prescription Medication ----(to be held and administered by an adult)
SBH-2 allows a student to have and carry prescription medication (inhalers, epi-pens, insulin, other medications).
----MOST people will only need to fill out the SHS-1 Form.
-Forms & Medications MUST be turned in Thursday Morning April 20th to the Directors at the Medicine Check in Table.
